Portfolio of Websites

This site was designed and built by CHTS for Audubon Surgery Center. The site includes a couple of forms that, once filled out online, are converted into a PDF file that is then emailed to Audubon Surgery Center where they can be saved along with other Electronic Medical Records or printed and filed. This site also uses the CHTS content management system.
